Third-Party Reproduction Psychological Evaluations
Bilingual, Evidence-Based Assessments for donors, recipients, intended parents, and gestational carriers
At Mind & Body Garden Psychology, we provide bilingual evidence-based psychological evaluations for individuals and families involved in assisted reproduction—including egg donation, sperm donation, embryo donation, and gestational surrogacy. All evaluations are conducted by licensed psychologists and follow guidelines established by the American Society for Reproductive Medicine (ASRM). Our psychologists speak both Mandarin and English.
Who Needs it?
A third-party reproduction evaluation or psychological consultation is highly recommended—and often required—for the following individuals:
Intended Parents (IPs) preparing for egg, sperm, or embryo donation
Egg Donors and Egg Donor Recipients
Sperm Donors and Sperm Donor Recipients
Gestational Carriers (GCs) and their partners
Anyone navigating infertility-related treatment and decision-making
What to Expect
The Evaluation Process Includes:
Comprehensive clinical interview (60–120 minutes)
Personality testing (300+ questions, 30-90 minutes)
Partner participation (when required by the referring agency or clinically appropriate)
Discussion of key areas:
Reproductive and mental health history
Family background, family history of conflict or significant crises, and support system
Any legal problems
Stress levels and demands on time
Attitudes toward third-party reproduction
Consent, boundaries, and future disclosure to children
Conducted via secure telehealth or in-person sessions.
All clients will receive consent forms and secure video links through our Simple Practice portal.

Complex-case policy & Add-on hours
If an evaluation requires additional sessions, collateral interviews, extended testing, or comprehensive addenda, time beyond the standard package is billed at $350 per clinical hour (pro-rated in 15-minute increments). Clients will be informed and must consent before any additional charges are incurred.
Written Report Delivery
Typical report delivery time frame: within 7 business days of completion, the written reports are delivered to your agency or referring clinic.
Rush reports: within 3 business days of completion, additional $150 flat fee will be charged.
Insurance & Payment
Fees are due at the time of service
We are out-of-network for all insurance plans
A superbill will be provided upon request for insurance reimbursement

Q & A:
Will my insurance cover the testing?
Many insurance plans cover psychological testing, but may not cover a 3rd-party psychological evaluation. Please contact your insurance to gather more information about reimbursement before the appointment for an assessment. If they don't cover it, then you will need to pay out of pocket.

Why Is a Psychological Evaluation Required in Third-Party Reproduction?
Intended Parent (IP) Counseling Session:
According to the American Society of Reproductive Medicine (ASRM), intended parents should attend counseling prior to an egg or sperm donation procedure. This is an opportunity for IPs to receive education, and discuss concerns so that they can comfortably and confidently approach their path to parenthood. This is a 60-90 minute counseling session that can be completed in person or by telehealth.
Donor Psychological Evaluation:
The American Society for Reproductive Medicine (ASRM) recommends that all prospective donors undergo a psychological evaluation during screening to limit the possibility of emotional harm. Psychological screening is generally aimed at identifying emotional problems, evaluating donor motivations, and verifying that women have a thorough understanding of the physical, psychological, and legal risks that could result from donation. The psychological evaluation includes the administration of psychological testing and an in-person or telehealth interview with the egg/sperm donor and their spouse or partner.
Donor Recipient Psychological Evaluation:
This evaluation aims to help the intended parents discuss how they feel about having a child not genetically related to one or both parents, and evaluate the level of their commitment and readiness psychologically. The psychological evaluation includes the administration of psychological testing and an in-person or telehealth interview with the egg/sperm donor recipient and their spouse or partner.
Gestational Carrier (GC) Psychological Evaluation:
The evaluation of a potential GC includes the administration of personality testing and a 2-3-hour structured clinical interview. The psychological interview includes questions outlined by the American Society for Reproductive Medicine (ASRM). If the GC has a spouse/partner, that individual also participates in part of the clinical interview. Your process is determined by the referring clinic or agency. This can be completed in person or by telehealth.
